The Youth In Transition Worker in an integral part of the RNJ Youth Services Team, supporting eligible youth who are connected with Family and Childrens Services with individualized assistance to help them transition from child to adult services. The Youth In Transition Worker accomplishes this responsibility through accessing, advocating and navigating service systems and community resources relevant to their needs, providing emotional support, life skills education and group facilitation.

 

Key Functions of the Position:

·       Determining program eligibility and fit through intake and assessment processes.

·       Connecting youth to supports and resources in the community including, housing, education, employment support, health, mental health, legal services and life skills training.

·       Providing holistic wrap around support to youth aging out of care to identify, access and navigate adult service systems relevant to their needs.

·       Fostering and maintaining strong relationships with referral agencies.

·       Coordinating services by attending networking meetings and community tables to stay informed of issues affecting the youth in Leeds and Grenville.

·       Develop and facilitate group workshops and presentations.

Job Qualifications:

·       College diploma or university degree in a related field.

·       Experience working with youth and facilitating groups.

·       Capacity to develop and nurture strong working relationships with community partners.

·       Exceptional oral and written communication skills including record keeping and statistics.

·       Flexible to meet the needs of clients and families and the agency.

·        A valid driver’s license and access to a reliable vehicle with required insurance

·       Standard First Aid, Mental Health First Aid and Non-Violent Crisis Intervention training are considered assets.

 

Successful candidates will be required to provide a satisfactory Criminal Record Check with Vulnerable Sector Screening as a condition of employment.
